This position assists individuals in public housing to address impairment related to behavioral health, health conditions, psychosocial need and/or housing barriers, and identifies resources and connects clients with behavioral health and other services.   Clinicians working in the BEHAVIORAL HEALTH OUTREACH PROGRAM excel at coordinating with external colleagues in a highly professional manner, have the opportunity to work in a self-directed capacity in the community for much of the week, and utilize a range of brief interventions and therapeutic clinical skills. As the program is project-based, we are able to engage clients outside of a fee-for-service model, but work diligently to connect individuals to long-term supports or providers when appropriate. With increased support for the BHOP program in 2022, we are looking to bring on team members who are proficient in brief therapy, clinical case management, crisis work, and have a SUD or MHP background.   Full time and part time positions available.   Current sign-on bonus: $5,000

The Forensic - SUD Clinician will work closely with the Courts and Probation to help the client meet their legal requirements. They may also outreach people in their homes, hospital or jail as needed. Interventions may be time limited or longer term, and will be part of an outcome-oriented treatment plan developed with in a managed care framework, which includes periodic reviews for appropriateness of services and intensity. Expanded Community Services-Clinician The Expanded Community Services (ECS) program is a community-based team that works with clients placed in Adult Family Homes (AFHs) throughout the community. Clients have both behavioral and medical issues with a history of lost placements. Clinicians are assigned to a region so that they are able to devote their time to working with clients and the AFH staff in a particular area. Clinicians on this team are in the community much of the time, setting their own daily schedules and agendas. In addition to working with individual clients there are also many opportunities to collaborate with facility staff, other providers, programs, systems and collaterals. Each ECS client has a state social worker who works in tandem with us.
