Sound Behavioral Health is growing and we are a behavioral healthcare navigator in Auburn, WA, supporting South King County! Sound is one of King County’s most comprehensive providers of quality mental health and addiction treatment services, helping our area’s most vulnerable populations. The behavioral healthcare navigator is responsible for coordinating care across a spectrum of behavioral health services. They provide assistance with obtaining Medicaid or Medicare coverage, understanding other insurance coverage, and initial screening and referral services to assist people in connecting to needed resources. They may also assist in developing a complete psychosocial evaluation, conducting initial health screenings, and developing a coordinated treatment plan. 

 

The behavioral healthcare navigator is an integral member of the clinic and provides care coordination, case management, and resource coordination. The navigator supports all people enrolled at the site, collaborates with other Sound providers and community providers as well as outside agencies; and adheres to agency, state, federal, and licensing board guidelines regarding ethical practice, billing, and documentation.

 

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Coordinates internal and external referrals.
  • Utilizes aspects of the SBIRT model (Screening, Brief Intervention, and Referral to Treatment), that can include referrals to other community partners.
  • Assists individuals and families with identifying and prioritizing goals
  • With individuals, formulate time-limited care coordination plans that target clearly identified care coordination goals with a means of measuring success and program completion.
  • Provides care coordination and resource connection
  • Establishes relationships with the community team, other providers, partnering community agencies, and identified support systems for clients, collaborating as indicated for clinical reasons.
  • Completes periodic reports as required by the agency and state.
  • Other duties as assigned.
  • The healthcare navigator will adhere to ethical principles about confidentiality, informed consent, compliance with relevant laws, and agency policies (e.g., critical incident reporting, HIPAA, Duty to Warn).

Establishes and maintains positive, ethical, and professional working relationships with clinical team members, clients, and outside organizations. Relates to a wide variety of people including members of ethnic and sexual minority populations, and is respectful of individual and cultural differences.

Qualifications

EDUCATION AND EXPERIENCE

  • Two years of experience in a healthcare or social services area of practice.
  • Bachelor's in social services or equivalent education and experience. 

 

KNOWLEDGE, ABILITIES, AND SKILLS

 

  • Possesses excellent judgment, effective communication and strong collaboration skills.
  • Understands recovery concepts in behavioral health; synthesizes and applies, as appropriate to their work at Sound.
  • Ability to learn new tasks and skills quickly, and to manage and prioritize multiple tasks, while simultaneously managing crises.
  • Possesses strong working knowledge of the physical and/or behavioral health and support resources of King County, and is familiar with state and local health delivery systems, including knowledge of various funding principles and clinical services mandates.
  • Able to demonstrate the capacity to appreciate and work well with the complex and diverse population.

 

CERTIFICATES, LICENSES & REGISTRATIONS

 

  • Washington State driver's license and insurable driving record.
  • Agency Affiliated Counselor Registration required upon hire.
  • Must pass a criminal background check. 

 

PHYSICAL DEMANDS

To successfully perform the essential duties of this position, an individual must be able to sit at a desk for four or more hours a day, and use office equipment, including phones and computer keyboards, for four or more hours per day. Individuals must be able to engage vendors, team members, clients and others over the phone and in-person by voice. Regular attendance is an essential function of the position. Compensation:
Bachelor's degree range: $29.94-$31.47 per hour
Master's degree range: $30.98-$33.64 per hour
