Overview

GENERAL SUMMARY
The CCBHC Registered Nurse Care Coordinator will provide assessment, education, stabilization, nursing care, care planning, and support to clients; and medical support to prescribing providers.

 

This particular position will work with the Federal Certified Community Behavioral Health Clinic (CCBHC) grant at our Auburn office.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide medical support to prescribing providers.
  • Conduct physical health assessments; and screening of clients interested in medication assisted treatment and those experiencing chronic medical conditions.
  • Provide client education about chronic health conditions, medication assisted treatment, medication compliance, etc.
  • Assist with coordination with pharmacy, primary care, medication refills and behavioral health team members.
  • Obtain vital signs, urine drug screens and basic labs.
  • Comply with documenting and reporting requirements.
  • Provide care planning to clients, and ongoing coordination of follow-up care.
  • Participate in stabilization and maintenance phases of medication assisted treatment of clients.
  • Provide relapse prevention support to clients.
  • Other duties as assigned at the discretion of management.

Qualifications

 EDUCATION and/or EXPERIENCE

  • Experience with psychiatric and substance use disorder client populations preferred
  • Experience with case management or care coordination preferred

 

KNOWLEDGE, SKILLS & ABILITIES

  • Competency at basic nursing skills such as venipuncture, IM injections, vital sign monitoring.
  • Basic computer literacy and familiarity with use of an electronic health record.
  • Familiarity with the use of Medication Assisted Treatment of substance use disorders.
  • Familiarity with community resources for behavioral health, substance use disorders, primary care.
  • Flexibility, team orientation. Strong internal motivation and work ethic.  Sound judgment.
  • Comply with agency policies and procedures.
  • Promptness and reliability in communication, attendance, work tasks and deadlines.
  • Active listening techniques that convey respect, professionalism, engagement, positivity, and empathy.
  • Maintains positive, ethical, and professional working relationships with clinical team members, clients, and outside organizations that are respectful of individual and cultural differences.

 

CERTIFICATES, LICENSES & REGISTRATIONS

  • A valid Washington State license as Registered Nurse. 
  • First Aid/CPR Certification required.
  • Washington State driver’s license and insurable driving record.
  • Must pass a criminal background check. 

 

PHYSICAL DEMANDS

To successfully perform the essential duties of this position, an individual must be able to sit at a desk for six or more hours a day, and use office equipment, including phones and computer keyboards, for six or more hours per day. Individuals must be able to engage vendors, team members, clients and others over the phone and in-person by voice. Regular attendance is an essential function of the position. Range: $51.00-53.60 per hour

 

Currrent Sign-On Bonus: $7,500
