We are looking for an amazing WISe Family Care Coordinator for our Lake City Way office who will act as a skill builder to teach and generalize needed skills that have been identified in the in the youth and family’s care plan. They will also provide case coordination and youth and family focused support to enrolled high-risk children/ youth and their caregivers in their homes and community using a wraparound model. Services and individualized safety principles promoted and taught in this position will assist family members in accessing and participating in community activities and resources, and mentoring and coaching positive communication, social, and parenting skills throughout the wraparound process. They will support the WISe team including our clinicians by providing coordinated behavioral health services. Hours are flexibly scheduled and based on family need. 

Responsibilities

ESSENTIAL FUNCTIONS AND DUTIES

  • Meet and engage the family communicating compassion, support, respect, confidence, and trust and enthusiasm for them and your role as a helper.
  • Provide non-judgmental, unconditional support and culturally relevant support to the child and family.
  • Work in collaboration and communicate effectively with WISe team staff and other service providers to provide seamless and helpful services.
  • Customize helping approaches to fit the family’s uniqueness, strengths, personality, culture and interest.
  • Provide necessary crisis response and stabilization services, completing and communicating the safety/crisis plan with the family and other team members.
  • Assists client to remain stable in the community, thereby reducing the use of hospitals or involuntary treatment services.
  • Coordinate and facilitate wraparound child and family team meetings, including development of the cross-system care plan.
  • Provide direct helping interventions that:
    • Respond to crises in a prompt, effective and collaborative manner.
    • Provide a range of therapeutic responses that support both the mental health treatment plan and cross system care plan.
    • Modify and adjust individual and family intervention techniques for each situation.
    • Provide skills building as needed to the youth and family.
    • Utilize parent/family strengths, goals, and vision in problem solving around specific needs.
    • Coach the family in effective ways to utilize the resources offered by formal and informal sources within their community.
  • Complete all documentation activities in keeping with the WISe Manual, King County or MCO requirements, agency policies, and State and Federal requirements.
  • Seek assistance and report concerns to the right people at the right time.
  • Inspire confidence in the child, family and other team members about their strengths and ability to succeed and transition successfully.
  • Understand where the family is in the Wraparound process and what actions need to occur during each phase.
  • Help the youth and family actualize the transition plan in preparation for transitioning out of WISe services.
  • Participates in WISe staff meetings, consultations, case presentations and in-service training.
  • Participates in ongoing program development and assessment.

KNOWLEDGE, ABILITIES, AND SKILLS

  • Has a practice level understanding of child and youth development and behavioral health interventions.
  • Understands wraparound principles and recovery concepts in behavioral health; synthesizes and applies, as appropriate to their work at Sound.
  • Demonstrates the ability to learn new tasks and skills quickly, and to manage and prioritize multiple tasks, while simultaneously managing crises.
  • Demonstrate ability to utilize technology.
  • Ability to demonstrate the capacity to appreciate and work well with the complex and diverse population.

Qualifications

EDUCATION AND EXPERIENCE

  • B.A. or B.S. in social sciences required.
  • One year of experience in a non-profit or a social service agency working with youth and families who need assistance, preferred.
  • Knowledge of youth development, family dynamics, effective interpersonal and behavioral management skills, and helping systems.

CERTIFICATES, LICENSES & REGISTRATIONS

  • Washington State driver’s license and insurable driving record required.
  • Must be able to obtain a WA State Agency Affiliated Counselor credential within the first 60 days of hire.
  • Must pass a criminal background check.

Current sign-on bonus: $3,000

Pay Range: $25.50 - $26.80 per hour
